The side effects of pioglitazone can be changes in blood sugar levels, including hypoglycemia, which is low blood sugar. Mild symptoms of hypoglycemia include lightheadedness, irritability, mood swings, numbness around the mouth, pallor, clumsiness, increased hunger and cravings, inability to concentrate and sweating. Moderate symptoms can involve increasing weakness, cold, clammy skin, increasing numbness around the mouth or fingers, a pounding heartbeat, memory loss, double vision and increasing confusion. A severe case of hypoglycemia can lead to unconsciousness and convulsions.

High blood sugar, or hyperglycemia, can also develop as a side effect of pioglitazone treatment. The symptoms can include fatigue, great hunger and thirst, an incessant need to urinate, and blurred vision. If hyperglycemia is untreated, it can lead to diabetic ketoacidosis, a dangerous condition where the body uses its own fat for fuel because it can no longer use glucose. As a result, waste products called ketones begin to concentrate in the body and acidify the blood. The symptoms are dehydration which leads to dry skin and dry mouth, breath that can smell fruity sweet, nausea and vomiting, a type of hyperventilation called Kussmaul breathing, headache, muscle and abdominal pain, and exhaustion. In the worst cases the patient becomes unconscious or slides into a coma, which can lead to death. Ketoacidosis is a medical emergency that most often requires hospitalization.

Other Side Effects
Other side effects of pioglitazone can be rhinorrhea, sore throat and other symptoms of a cold, headaches, and pain in the muscles, teeth or mouth. The doctor should be told of these symptoms when convenient. However, the doctor should be called immediately for symptoms like nausea, vomiting, fatigue, jaundice, which presents as yellow eyes, yellow skin and dark urine, and disturbances in vision.

Also, some patients who took pioglitazone had an increased risk for bladder cancer, and women who took the drug had increased incidence of bone fractures in the limbs and extremities, though the risk of increased fractures wasn’t true for men.
